About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Lincolnshire County Council. As a Support Worker, you will play a vital role in supporting individuals with disabilities to live fulfilling lives.

Key Responsibilities
  • Provide person-centered support to individuals with disabilities, promoting their independence and well-being.
  • Develop and implement personalized care plans, working closely with individuals, their families, and other healthcare professionals.
  • Assist with daily living tasks, such as personal care, meal preparation, and household chores.
  • Support individuals to participate in activities and hobbies that promote their physical and mental health.
  • Work collaboratively with other healthcare professionals to ensure seamless care and support.
Requirements
  • Experience working with individuals with disabilities, preferably in a care or support setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and colleagues.
  • Ability to work flexibly, including evenings, weekends, and public holidays.
  • Commitment to promoting independence, dignity, and well-being for individuals with disabilities.
