Autonomous Practitioner

We are seeking an experienced and skilled Autonomous Practitioner to join our Hospital and Care at Home (H&C) team. As a key member of the multi-disciplinary team, you will work closely with patients, carers, and healthcare professionals to provide high-quality, patient-centered care in the comfort of their own homes.
Key Responsibilities- Assess patients' needs and develop personalized care plans using evidence-based clinical skills and digital technology.
- Work autonomously, without direct supervision, to deliver high-quality care and make clinical decisions.
- Collaborate with medical colleagues and the wider multidisciplinary team to ensure seamless care transitions and effective communication.
- Provide clinical leadership and guidance to the H&C team, ensuring safe and effective care delivery within available resources.
- Contribute to the development and implementation of clinical policies, procedures, and protocols to enhance patient care and outcomes.
- Participate in education and training activities to enhance the skills and knowledge of the H&C team and other healthcare professionals.
