About the Role

We are seeking an experienced and skilled Autonomous Practitioner to join our Hospital and Care at Home (H&C) team. As a key member of the multi-disciplinary team, you will work closely with patients, carers, and healthcare professionals to provide high-quality, patient-centered care in the comfort of their own homes.

Key Responsibilities
  • Assess patients' needs and develop personalized care plans using evidence-based clinical skills and digital technology.
  • Work autonomously, making decisions and taking actions without direct supervision, drawing on advanced theoretical and practical experience.
  • Collaborate with medical colleagues and the wider multidisciplinary team to ensure seamless care transitions and effective communication.
  • Provide specialist advice and support to individuals or the multidisciplinary team in the care of patients within the H&C service.
  • Contribute to the development and delivery of formal and informal education to members of the multidisciplinary team.
  • Ensure compliance with professional policies, national and local guidelines, and Trust policies, procedures, and protocols.
