Autonomous Practitioner

We are seeking an experienced and skilled Autonomous Practitioner to join our Hospital & Care At Home team as a Virtual Ward Clinical Lead. This is an exciting opportunity to work in a dynamic and innovative environment, providing high-quality care to patients in their own homes.
Key Responsibilities- Work as an autonomous practitioner, assessing patients' needs and providing evidence-based clinical care using digital technology.
- Lead the Virtual Ward team, ensuring safe and effective care delivery, and making clinical and managerial decisions as needed.
- Develop and implement care pathways, working closely with multidisciplinary teams to ensure seamless patient care.
- Provide specialist advice and support to patients, carers, and healthcare professionals, promoting holistic and rapid assessment of patients.
- Manage complex patient cases, working collaboratively with medical colleagues and the wider multidisciplinary team.
- Contribute to the development of formal and informal education programs for healthcare professionals, sharing expertise and best practices.
- Current NMC or AHP registration.
- Degree or Diploma in Nursing or AHP.
- Previous experience in a speciality, with a strong background in clinical and managerial skills.
- Evidence of Continuing Professional Development (CPD).
- Enhanced clinical skills qualification or willingness to undertake.
- NMP or prepared to undertake.
