Autonomous Practitioner

The Northern Care Alliance NHS Foundation Trust is seeking a highly skilled and experienced Autonomous Practitioner to join our Hospital and Care at Home team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to patients in their own homes, utilizing evidence-based clinical skills and digital technology.
Key Responsibilities- Assess patients' needs and develop personalized care plans, working within agreed protocols and guidelines.
- Deliver high-quality care to patients, ensuring timely and effective interventions to prevent clinical deterioration.
- Collaborate with medical colleagues and other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Manage complex patient caseloads, prioritizing and planning workload to ensure efficient and effective care delivery.
- Develop and implement education and training programs for healthcare professionals, promoting best practices and evidence-based care.
- Contribute to the development and implementation of clinical policies and procedures, ensuring compliance with Trust standards and guidelines.
- Current registration with the NMC or AHP register.
- Degree or Diploma in Nursing or AHP.
- Previous experience in a speciality, with a minimum of 4 years' post-registration experience.
- Evidence of continuing professional development and a commitment to ongoing learning.
- Enhanced clinical skills qualification or willingness to undertake.
- NMP or prepared to undertake.
