Key Responsibilities- Assess patients' needs and develop personalized care plans using evidence-based clinical skills and digital technology.
- Work autonomously, making decisions and taking actions without direct supervision, drawing on advanced theoretical and practical experience.
- Collaborate with medical colleagues and the wider multidisciplinary team to ensure seamless care transitions and effective communication.
- Provide specialist advice and support to individuals and the multidisciplinary team, utilizing advanced knowledge and skills to deliver high-quality care.
- Manage complex patients and prioritize workload effectively, ensuring timely and efficient care delivery.
- Contribute to the development and delivery of formal and informal education to members of the multidisciplinary team.
