Job Summary

The Department of Pediatrics at the University of Virginia School of Medicine is seeking a dedicated RN Case Manager/Clinical Nurse to provide relationship-based care within a collaborative multidisciplinary team, ensuring optimal patient outcomes.

Responsibilities:
  • Develop and implement individualized care plans, prioritizing patient and family needs.
  • Establish and maintain effective relationships with patients, families, colleagues, and interdisciplinary team members to enhance the patient experience.
  • Conduct clinical assessments, coordinate care, and implement interventions to promote healing and alleviate suffering.
  • Demonstrate leadership by actively participating in shared governance and supporting the onboarding and precepting of new team members.
  • Engage in lifelong learning through professional development activities, staying updated on best practices and evidence-based interventions.
  • Adhere to clinical documentation guidelines, analyze outcomes data, and participate in performance improvement activities to ensure quality achievement.
  • Embrace innovation by utilizing technology to support patient care and implementing evidence-based practices derived from nursing research.
Requirements:
  • Education: Graduate of accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire.
  • Experience: One year of relevant experience required.
  • Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required.
Physical Demands:
  • Job requires standing for prolonged periods, frequently traveling, and bending/stooping.
  • Proficient communicative, auditory, and visual skills; Attention to detail and ability to write legibly.
  • Ability to lift/push/pull 20 - 50lbs.
  • May be exposed to chemicals, blood/body fluids, and infectious diseases.
Anticipated Hiring Range:

$60,000 - $90,000 based on education and experience.

This position is located in Bristol, VA.
