Position Overview
We are seeking a dedicated and compassionate Clinical Nurse Specialist to become a vital member of our collaborative healthcare team at Concord Medical Centre. Our practice is committed to delivering exceptional patient care and fostering a supportive environment for both staff and patients.
Key Responsibilities
- Conduct comprehensive assessments, screenings, and treatment services while providing health education to patients.
- Administer nursing interventions in collaboration with physicians or independently, adhering to established protocols.
- Perform general and specific health screenings for patients, ensuring appropriate referrals to physicians when necessary.
- Engage in health promotion initiatives for patients with chronic conditions, including diabetes, asthma, and cardiovascular diseases, following established practice guidelines.
- Oversee the management of the leg clinic and facilitate the leg management pathway.
- Assist in the preparation and administration of vaccinations, immunizations, and travel health programs.
- Collect pathological specimens, including blood samples and swabs, and perform investigatory procedures as directed by physicians after receiving appropriate training.
- Lead initiatives in chronic disease management.
- Maintain accurate and detailed records of all patient consultations and treatments in the electronic medical records system.
- Contribute to the collection of data for audit projects and assist in evaluating and presenting findings as required.
- Participate in research opportunities within the practice and the broader Primary Care Network, collaborating with our research team supported by academic institutions.
- Engage in the Covid medicine delivery unit, providing infusions for vulnerable populations requiring treatment.
