Position Overview

The successful candidate will deliver comprehensive healthcare services to patients registered at Gloucester Road Medical Centre. This role involves collaboration within a dedicated treatment room team, where you will manage a variety of scheduled appointments catering to both adult and pediatric populations.

We are looking for a Clinical Nurse Specialist for either a part-time or full-time role to provide essential nursing services. Experience in a primary care setting is preferred, particularly in the management of patients with chronic respiratory conditions such as Asthma and COPD, which are currently areas of need within our team.

Key Responsibilities

  • Collaborate with the nursing team to administer nursing care to all patients registered with the practice.
  • Engage with other healthcare professionals to achieve established targets aligned with quality standards and national guidelines.
  • Supervise junior staff as necessary.
  • Maintain accurate and thorough nursing documentation, including electronic records, in accordance with practice policies.
  • Adhere to the NMC Code of Professional Conduct and its principles.
  • Follow established clinical protocols, consulting with senior nurses or GPs as needed.
  • Perform wound management, including compression bandaging, following appropriate training and assessment.
  • Conduct ECGs and venepuncture as required.
  • Carry out cervical screening and basic sexual health assessments, following necessary training.
  • Administer travel and routine immunizations as needed, utilizing Patient Group Directions (PGDs).
  • Request pathology tests, including swabs and samples.
  • Engage in health promotion activities, including weight management and smoking cessation, while directing patients to the appropriate healthcare practitioners.
  • Manage patients with long-term respiratory conditions, dedicating approximately 30% of your working time to this area.

Clinical Responsibilities

Long-Term Condition Management:

  • Oversee and treat patients with Asthma and COPD, including routine assessments and management of exacerbations.
  • Conduct annual medication reviews, patient education, and diagnostic planning.
  • Utilize various consultation methods, including telephone, face-to-face, and virtual platforms.
  • Facilitate referrals to secondary care when necessary.
  • Ensure clinical practices are safe, effective, and within the scope of competence.

Treatment Room Duties:

  • Perform ECGs, venepuncture, wound care, and routine immunizations in accordance with national guidelines.
  • Assist patients during consultations with other clinicians as needed.
  • Request necessary pathology tests, including blood tests and swabs.
  • Provide contraception advice and emergency contraception within competency limits, with support from GPs and Nurse Manager.

Administrative and Professional Responsibilities

  • Ensure the clinical computer system is updated with accurate patient information.
  • Participate in practice meetings as required.
  • Maintain and restock clinical areas and consulting rooms.
  • Deliver evidence-based care at the highest standards to ensure quality patient care.
  • Adhere to local policies and procedures.
  • Engage in audit processes and implement practice changes as necessary.
  • Contribute positively to team effectiveness and efficiency.

Training and Development

Training needs will be assessed through annual appraisals and interim reviews, aligned with practice requirements. Continuous education through relevant courses and study days will be supported, along with participation in any training programs implemented by the practice.

Qualifications

Essential:

  • Degree or postgraduate qualification in nursing.
  • Current professional registration.
  • Evidence of recent continuing professional development.
  • Two years of recent experience in acute or primary care.
  • Recent experience in direct patient care.

Desirable:

  • Recognized prescribing qualification.
  • Experience in primary care settings.
