We are seeking a skilled Rehabilitation Case Coordinator to join our team. As a Rehabilitation Case Coordinator, you will be responsible for coordinating and managing the rehabilitation process for clients with minor to catastrophic life-changing injuries.
Key Responsibilities:
- Coordinate and manage the rehabilitation process for clients, including developing and implementing individualized rehabilitation plans.
- Work closely with healthcare professionals, including doctors, therapists, and other specialists, to ensure comprehensive care and support for clients.
- Provide case management services, including assessing client needs, developing care plans, and coordinating services to meet those needs.
- Monitor client progress and adjust rehabilitation plans as needed to ensure optimal outcomes.
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care and support.
Requirements:
- Strong knowledge of rehabilitation principles and practices.
- Excellent communication and interpersonal skills.
