An exceptional opportunity has emerged within the Manchester University NHS Foundation Trust for a Band 7 Palliative Care Nurse Consultant in the Community Macmillan Team.

The Palliative Care Nurse Consultant will collaborate as part of a comprehensive multidisciplinary team dedicated to providing Palliative Care Support. The individual in this role will assist local teams in delivering high-quality, patient-centered care that is responsive to the needs of patients and their families. This includes the education of healthcare professionals, as well as offering training, support, and guidance to the multidisciplinary team. The primary aim of this collaborative approach is to empower patients and their families to maintain a good quality of life, avoid unnecessary hospital admissions, and fulfill their care preferences.

Key Responsibilities

  • Deliver palliative care aimed at enhancing the quality of life for patients and families facing complex physical, psychological, and emotional challenges.
  • Take charge of the specialized nursing assessment and treatment of clients referred to the Palliative Support Service.
  • The Nurse Consultant will be responsible for triaging all referrals for the multidisciplinary team and providing palliative care guidance to patients, families, and other healthcare professionals during the triage process.
  • The Nurse Consultant will be expected to visit patients requiring urgent palliative care support, utilizing advanced communication skills and collaborative efforts with other teams.
  • Collaborate within a multidisciplinary team to conduct comprehensive assessments and interventions for clients with both complex and non-complex cancer needs, as well as those with non-cancer diagnoses.
  • Manage a caseload of patients needing palliative care input, ensuring that all staff members deliver high-quality care. Supervise Associate Macmillan Nurses to ensure competency in supporting patients within the caseload. Provide oversight and clinical support to band 4 Assistant Practitioners.
