About the Role

We are seeking a highly skilled and experienced Patient Pathway Coordinator to join our team at Swansea Bay University Health Board. As a key member of our Patient Pathway Team, you will play a vital role in supporting the delivery of our Referral To Treatment targets and Service and Financial Framework targets.

Main Responsibilities

  • Liaise with staff groups across the Health Board on behalf of the Surgical Specialties Directorate, providing and receiving routine information and distributing it to relevant staff.
  • Highlight and communicate any risk of breaches to clinicians and managers to enable further action to be taken.
  • Utilize patient management systems to analyze and report on healthcare data.
  • Collaborate with healthcare professionals to ensure effective patient pathway management and waiting list validation.

Requirements

  • GCSEs grade C and above.
  • NVQ Level 3 Business Admin.
  • Experience of healthcare systems, previous NHS background, and experience in information analysis.
  • Ability to use patient management systems, knowledge of RTT guidelines, and knowledge of health board values.
