Community Support Facilitator

Position Overview
Catalyst Support, in collaboration with Surrey and Borders Partnership NHS Foundation Trust, delivers essential Liaison and Diversion services to the community. Our mission is to provide proactive assistance to vulnerable individuals entangled in the criminal justice system. We conduct thorough evaluations to identify those facing mental health challenges, learning disabilities, substance misuse issues, and other vulnerabilities, ensuring they receive the necessary support and referrals to suitable services.
Insights gained from these evaluations are shared with pertinent youth and criminal justice organizations to facilitate informed decision-making regarding diversion, charging, case management, participation in legal processes, remand, and sentencing for these individuals.
The Community Support Facilitator works closely with team members in the Liaison and Diversion Service to deliver individualized community support and interventions, enhancing health and social care while minimizing reoffending and promoting effective decision-making within criminal justice collaborations.
Key Responsibilities
- Engage with individuals in criminal justice environments following assessments
- Assist clients in their local communities to achieve personal objectives
- Conduct ongoing risk evaluations and formulate risk management strategies
- Comply with the policies of the Liaison and Diversion service
- Offer guidance and suitable harm reduction techniques
- Support clients in accessing vital local services for health enhancement
- Collaborate with fellow support workers to meet legal obligations
- Provide transportation to appointments and ongoing assistance as required
- Establish productive relationships with local services and participate in joint assessments and partnership discussions
- Help individuals secure employment, training, and access to support services
- Maintain accurate records and data in accordance with service agreements

Role Summary
In partnership with Surrey and Borders Partnership NHS Foundation Trust, Catalyst Support provides vital Liaison and Diversion services to the community. This service emphasizes early intervention for vulnerable individuals involved with the criminal justice system, offering support, guidance, and referrals tailored to their specific needs. Information collected from assessments is shared with relevant youth and criminal justice agencies to aid informed decision-making. The Community Support Facilitator collaborates with colleagues to deliver individualized community support, enhancing health and social care while reducing reoffending within criminal justice partnerships.

Qualifications
Desirable
- A diploma or degree in a relevant field (counseling, psychology, social work, etc.) is advantageous but not essential
Experience
Essential
- Experience in community settings working with adults facing mental health challenges, learning difficulties, substance misuse, and housing issues
- Ability to engage with complex clients in community settings
Desirable
- Familiarity with assertive outreach practices, risk assessment, safeguarding, and relevant policies
- Understanding of harm reduction methodologies
