Position Overview:
Our client, a dedicated non-profit organization, is committed to enhancing the health and wellbeing of individuals in the community. With over three decades of experience, they collaborate with various statutory and voluntary organizations to deliver impactful services in the Surrey region.

Role Summary:
They are seeking a compassionate and organized Community Outreach Support Specialist to join their team within the Surrey Liaison and Diversion Service. This role involves providing outreach support across the Surrey community and engaging with individuals in local prisons to assess their needs and offer support prior to their release.

Key Responsibilities:
- Deliver outreach support to clients in the community, including visits to various locations.
- Collaborate with the Liaison and Diversion Service team to provide person-centered care and interventions.
- Conduct assessments to identify vulnerabilities such as mental health issues, substance misuse, and housing concerns.
- Facilitate referrals to appropriate support services and assist clients in accessing necessary resources.
- Maintain strong communication with multidisciplinary teams to ensure comprehensive care.

Salary and Benefits:
- Competitive salary of up to £27,000 per annum, based on experience.
- 35 hours per week.
- Permanent position.

Work Environment:
The role involves a blend of community-based work and some prison engagement, focusing on building relationships with individuals in need of support. Ideal Candidate:
The successful applicant will possess:
- A strong commitment to delivering excellent person-centered care.
- A willingness to learn and enhance the quality of support provided.
- Experience in community settings, particularly with individuals facing mental health challenges, learning disabilities, or substance misuse issues.
- Knowledge of the criminal justice system and health and social care practices is advantageous.
- Effective communication skills, both written and verbal.
- Proficiency in Microsoft Office applications and familiarity with electronic patient record systems.
- Flexibility and willingness to travel as required.

This position is subject to an enhanced DBS check and full vetting procedures.
