Role Summary: They are currently seeking a compassionate and organized Community Support Specialist to join their team within the Liaison and Diversion Service. This role focuses on providing outreach support to individuals in the community.
Work Schedule: 35 hours per week, Monday to Friday. This is a permanent position.

Travel Requirements: The role involves some travel to meet clients across the community, with travel expenses reimbursed. A valid driver's license is preferred.
Key Responsibilities: The Community Support Specialist will collaborate with colleagues in the Liaison and Diversion Service to provide person-centered and community-based support. The aim is to enhance health and social care outcomes for individuals, ultimately working towards reducing recidivism.
Service Focus: The Liaison and Diversion Service is dedicated to improving the lives of vulnerable individuals, offering early intervention for those aged 10 and above who engage with the criminal justice system.
Assessment and Support: The team conducts assessments to identify various vulnerabilities, including:
- Mental health issues
- Physical health concerns
- Learning disabilities
- Substance misuse
- Housing and financial difficulties
Based on these assessments, the Community Support Specialist will provide tailored support, which may include:
- Signposting to relevant services
- Referrals to appropriate support networks
- Assistance with appointments
- Collaborative efforts with other organizations
- Holistic wellbeing support
Requirements: Candidates should possess:
- A passion for delivering exceptional person-centered care
- A willingness to learn and enhance care quality
- Experience in community settings, particularly with mental health, learning difficulties, substance misuse, or homelessness
- Knowledge of the criminal justice system and/or health and social care
- Familiarity with assertive outreach practices, risk assessment, and safeguarding policies
- Proficiency in Microsoft Office applications and experience with electronic patient record systems
