Position Overview:

With over three decades of experience, they have collaborated with various statutory and voluntary, community, and social enterprise (VCSE) partners in the Surrey region.

Currently, they are seeking a Community Support Facilitator to join the Community Connections team, providing support to clients throughout the Guildford and Waverley areas.


Compensation:
£25,440

Working Hours: 35 hours per week

The Community Connections initiative is a collaborative contract funded by Clinical Commissioning Groups and Surrey County Council, working alongside various partners to provide a comprehensive range of mental health support and wellbeing activities.

This service is available to adults seeking to enhance or maintain their sense of wellbeing.

Individuals looking for assistance to improve their mental health and wellbeing or to prevent the deterioration of existing mental health issues can access this service.


Role Responsibilities:
The primary function of this role is to empower individuals to take charge of their health and wellbeing, promoting healthier lifestyles and increasing awareness of the importance of 'wellbeing' and sound mental health within an integrated care framework.

You will engage in delivering community-based activities and interventions, including one-on-one support, group sessions, and wellbeing initiatives, which complement and reduce the reliance on services provided by the statutory sector.

This position is subject to a DBS check. Please note that a history of substance use or criminal activity will not automatically disqualify you from this role.
Ideal Candidate Profile:
  • Previous experience in a mental health or wellbeing role supporting adults
  • Ability to collaborate effectively within a multidisciplinary team
  • Understanding of safeguarding policies and practices
  • Extensive knowledge of mental health issues
  • Skills in engagement, motivation, and behavior change techniques
  • Strong written and verbal communication skills
  • Proficiency in Microsoft Word and Excel
  • Willingness to travel and work flexibly
