About the Role

We are seeking a highly skilled and experienced Consultant Geriatrician to join our Community Frailty Service team at Kent Community Health NHS Foundation Trust.

Key Responsibilities
  • Provide high-quality clinical care to patients with frailty and complex needs
  • Work collaboratively with a multidisciplinary team to develop and implement effective care plans
  • Contribute to the development and delivery of community-based services for patients with frailty
  • Participate in quality improvement initiatives to enhance patient outcomes and experience
  • Engage in education and training to maintain expertise and share knowledge with colleagues
Requirements
  • Professional qualifications in Geriatric Medicine (MRCP, MRCPCH or equivalent)
  • Full GMC registration and CCT accreditation in Geriatric Medicine
  • At least 2 years of Higher Specialist Training in Geriatric Medicine or equivalent experience
  • Excellent clinical skills, communication, and interpersonal skills
  • Ability to work effectively in a team and lead change initiatives
