About the Role

We are seeking a highly skilled and experienced Geriatrician Specialist to join our Community Frailty Service team at Kent Community Health NHS Foundation Trust. As a Consultant Geriatrician, you will play a key role in providing high-quality patient care and contributing to the development of our service.

Key Responsibilities

  • Provide comprehensive geriatric assessments and develop individualized care plans for patients with frailty and complex needs.
  • Work closely with multidisciplinary teams, including GPs, advanced frailty practitioners, and other healthcare professionals, to ensure seamless care coordination.
  • Contribute to the development and implementation of service improvements, including the expansion of our Community Hospitals and Primary Care Network (PCN) Hubs.
  • Participate in education and training initiatives to enhance the skills and knowledge of our team members.

Requirements

  • Professional qualifications (MRCP, MRCPCH by examination or equivalent) and CCT accreditation within the area of speciality.
  • At least 2 years Higher Specialist Training within the area of speciality or equivalent experience.
  • Excellent clinical skills, ability to manage time and priorities effectively, and strong leadership and co-ordination skills.
