About the Role

We are seeking a highly skilled Consultant Geriatrician to join our Community Frailty Service team at Kent Community Health NHS Foundation Trust. As a key member of our team, you will be responsible for providing high-quality care to patients with frailty, working closely with other healthcare professionals to ensure seamless delivery of services.

Main Responsibilities

  • Develop and implement effective care plans for patients with frailty, taking into account their individual needs and circumstances.
  • Work collaboratively with other healthcare professionals, including GPs, advanced frailty practitioners, and trainee ACPs, to ensure comprehensive care for patients.
  • Contribute to the development and implementation of service improvements, including the expansion of our Community Frailty Service.
  • Provide education and training to other healthcare professionals on frailty care and management.

Requirements

  • Professional qualifications (MRCP, MRCPCH by examination or equivalent) and CCT accreditation within area of speciality.
  • At least 2 years Higher Specialist Training within area of speciality, including experience in Community Paediatrics.
  • Excellent clinical skills, ability to manage time and priorities effectively, and strong leadership and co-ordination skills.
