About the Role

This is an exciting opportunity to join the Dorset Clinical Commissioning Group as a Healthcare Support Worker, specializing in Palliative Care. As a key member of our team, you will play a vital role in providing high-quality care to patients with acute and long-term conditions.

Key Responsibilities

  • Provide person-centered care to patients with complex needs, working closely with the multidisciplinary team to ensure seamless care delivery.
  • Assist in the administration of medications, including non-IV and IV medications, and perform vital signs monitoring.
  • Develop and maintain effective communication skills to engage with patients, carers, and healthcare professionals.
  • Contribute to the development and implementation of care plans, ensuring patient-centered and evidence-based practice.
  • Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.

Requirements

  • NMC Registered Nurse with a strong background in Adult Health Nursing Studies.
  • Advanced Diploma or Degree in Adult Health Nursing Studies.
  • Experience in caring for patients with acute and long-term conditions, with a focus on End of Life Care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Basic IT skills and proficiency in vital signs monitoring equipment.

Desirable Criteria

  • Teaching Qualification and experience in education and training.
  • Post-registration courses related to Palliative Care and End of Life Care.
  • Leadership and management skills, with experience in supervising and mentoring staff.
