Senior Integrated Community Children's Nurse

About the Role
Solent NHS Trust is seeking a highly skilled and experienced Senior Integrated Community Children's Nurse to join our team. As a Senior CCN, you will play a key role in shaping and leading our responsive service, providing high-quality, family-centered care to children and young people with complex health care needs and disabilities.
Key Responsibilities
- Manage a caseload of children and young people with complex health care needs, providing specialized, high-quality nursing care and developing individualized programs of care.
- Support the management of integrated acute and community care pathways for children with short-term conditions.
- Provide supervision and monitoring of junior staff, with support from the team leader.
- Work with families to provide emotional support and specialist clinical care, sensitive to the changing and ongoing needs of the child, siblings, parents, and other carers.
- Teach families/carers to carry out specific complex nursing care and procedures, enabling them to feel confident and competent in caring for the child.
Requirements
- BSc Nursing or RN in Child or Learning Disability Nursing, with current registration with the NMC and CCN Specialist Practitioner Qualification or willingness to complete within 2 years.
- Experience in assessing and developing patient care plans, developing and delivering care and clinical interventions, and MDT working.
- Clinically competent at Band 6 level, with excellent communication skills, written and verbal, and ability to work autonomously, prioritize workload, and instruct and direct others.
