Integrated Children's Community Nurse

We are seeking a highly skilled and experienced Senior Community Children's Nurse to join our Integrated Children's Community Nursing (iCCN) team at Sussex Community. As a key member of our team, you will play a vital role in providing high-quality, family-centered care to children and young people with complex health care needs and disabilities.
Main Responsibilities- Provide specialist nursing care, support, and coordination of care to children and young people with complex health care needs and disabilities.
- Work collaboratively with children, young people, families, and the wider multi-disciplinary team to ensure care is provided as close to home as possible.
- Develop and deliver care and clinical interventions to patients, including those with acute and short-term conditions, long-term conditions, disabilities, and life-limiting and life-threatening illness.
- Teach families/carers to carry out specific complex nursing care and procedures, enabling them to feel confident and competent in caring for the child.
- Supervise and monitor junior staff with the support of the team leader.
- BSc Nursing or RN in Child or Learning Disability Nursing and currently registered with NMC CCN Specialist Practitioner Qualification or willingness to complete within 2 years of being appointed.
- Experience in assessing and developing patient care plans, developing and delivering care and clinical interventions to patients, and teaching, training, and supervision/assessing junior staff and students.
- Clinically competent at Band 6 level, with teaching, supervision, and assessing skills, and understanding of the health needs of people in their care and health promotion.
- UK Driving Licence and use of a car to carry out community visits.
