Senior Integrated Community Children's Nurse

We are seeking a highly skilled and experienced Senior Integrated Community Children's Nurse to join our team at Solent NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality nursing care to children and young people with complex health needs.
Key Responsibilities- Provide specialist nursing care to children with a range of health conditions, including long-term conditions, complex needs, and life-limiting illnesses.
- Work collaboratively with other healthcare professionals to develop and implement individualized care plans.
- Conduct comprehensive assessments and evaluations to identify the needs of children and their families.
- Develop and implement programs of specialized nursing care to meet the needs of children and their families.
- Provide education and support to children, young people, and their families to promote health and well-being.
- Participate in clinical audits and quality improvement initiatives to ensure high-quality care.
- Contribute to the development and implementation of policies and procedures related to children's health care.
- Registered Children's Nurse (RSCN) or equivalent qualification.
- Post-registration qualification in Foundation in History Taking and Assessment or equivalent.
- Specialist CCN degree or equivalent.
- Teaching/Mentor qualification.
- At least 2 years of experience in community children's nursing.
- Experience of working collaboratively with other healthcare professionals.
- Experience of supporting children and families with complex health care needs.
