About the Role

We are seeking a compassionate and empathetic individual to join our Mountbatten Care at Home Team as a Caregiver. As a key member of our team, you will provide high-quality care and support to patients with life-limiting conditions, their families, and carers in the comfort of their own homes.

Key Responsibilities

  • Deliver direct care, treatment, and emotional support to patients as required within set protocols and policies.
  • Contribute to the assessment, implementation, and monitoring of patients and carers, identifying physical and/or psychological problems and liaising with the appropriate health and social care team members.
  • Work collaboratively with colleagues to ensure the delivery of high-quality, safe, and compassionate care and support.
  • Promote and maintain effective liaison with the care team and other health and social agencies involved in the patients' care.
  • Assist in the day-to-day organisation of the service, carrying out duties as delegated by the Care at Home Team Leader and Clinical Nurse Specialist.

Requirements

  • Experience of dealing with vulnerable adults in a care setting.
  • Ability to work shift patterns on a rota basis, including nights, weekends, and bank holidays.
  • Full driving license and use of vehicle for work purposes with business insurance.
  • NVQ level 2 in healthcare or equivalent, or willingness to work towards.
