We are seeking a highly skilled and compassionate Rehabilitation Assistant to join our team at Mountbatten Hampshire. As a key member of our Rehabilitation and Enablement Team, you will play a vital role in delivering high-quality, patient-centered care to individuals with life-limiting conditions.
Key Responsibilities- Deliver skilled assessments and therapeutic interventions to patients, promoting a palliative rehabilitation and enablement approach.
- Manage and plan your caseload effectively, reporting back to the qualified therapists and multidisciplinary team to ensure sound communication and collaboration.
- Support people with progressive life-limiting illness to optimize their function and wellbeing, enabling them to live as independently and fully as possible with choice and autonomy.
- Champion and promote a rehabilitation and enablement approach across all Mountbatten services, embracing positive risk-taking.
- Attend regular staff, open, and information exchange meetings.
- Respect and guide volunteers working in the rehabilitation team.
- Share responsibility for indirect patient contact tasks, such as answering telephones, arranging appointments, and processing referrals.
- Formal technical qualification, e.g., NVQ3 in a healthcare-related subject or equivalent.
- Minimum of 2 years' previous experience in a therapy assistant role.
- Experience of working with people in a caring and supportive role.
- Computer literacy skills.
- Good organizational skills.
- Good verbal and written communication skills.
- Observation skills.
