Job Title: Advanced Care Practitioner

We are seeking a highly skilled and experienced Advanced Care Practitioner to join our team at Eden Medical Group. As an Advanced Care Practitioner, you will play a vital role in providing high-quality patient care and contributing to the development of our Home Visiting Service.

Key Responsibilities:
  • Provide clinical support to the PCN Frailty Team and work closely with the PCN Leadership Team and wider Health and Care Partners.
  • Develop and implement evidence-based clinical practice, supported by continual professional development, as part of the wider multi-disciplinary team.
  • Work collaboratively with the PCN Team, including ACPs, Nurses, Frailty Care Co-ordinators, Social Prescribers, Pharmacy Technicians, and Long-term Conditions Co-ordinators.
  • Provide patient-centered care, primarily through the Home Visiting Service, and work closely with GP Practices to ensure seamless care delivery.
  • Assess patients with undifferentiated and undiagnosed problems, initiate treatment, and provide advice and management as needed.
  • Prescribe safe, effective, and appropriate medication as defined by current legislative framework.
  • Work with the medicines management team to provide and/or ensure polypharmacy medication reviews and de-prescribe where appropriate.
  • Contribute to the development and delivery of a high-quality innovative service to patients within the community.
Requirements:
  • Current professional registration as an Advanced Care Practitioner.
  • Independent Non-Medical Prescriber V300 qualification.
  • Evidence of recent Continual Professional Development.
  • Leadership skills and experience working at clinical practitioner level.
  • Ability to assess and manage patient risk effectively and safely.
  • Excellent interpersonal, verbal, and written communication skills.
  • Reflective practitioner with a commitment to lifelong learning.
