About the Role:

We are seeking a highly skilled and experienced Care Coordinator to join our team at Pin Point Health & Social Care. As a Care Coordinator, you will play a vital role in supervising your own team, coordinating service user packages, and rostering staff to ensure excellent continuity and team building.

Key Responsibilities:

  • Supervise and manage a team of care professionals in a specific area, providing guidance and support to ensure high-quality care delivery.
  • Coordinate service user packages, including rostering staff, to ensure seamless care delivery and continuity.
  • Conduct service user reviews, outcome assessments, and provide supervision and probationary support to new staff members.
  • Participate in on-call work on a rota basis, including weekends and evenings, to ensure 24/7 care delivery.

Working Hours:

Monday to Friday, with flexibility to work additional hours as required to ensure care cover.
