Job Summary:
We are seeking a skilled and compassionate Diversity and Inclusion Lead to join our team at Inclusion Recovery Hampshire. As a key member of our multidisciplinary team, you will play a vital role in ensuring a safe and positive environment for our service users.
Key Responsibilities:
- Collaborate closely with healthcare professionals, managers, and allied health colleagues to deliver comprehensive care and support.
- Conduct thorough assessments of service users to develop tailored treatment plans, including medication management and health interventions.
- Prescribe and review medication, adhering to best practices guidelines and ensuring the highest standards of care.
- Provide guidance and support to service users, promoting their health and wellbeing.
- Facilitate blood-borne virus screening and vaccinations, contributing to the overall health improvement of our service users.
- Maintain accurate and up-to-date records of assessments, interventions, and treatment plans.
