As a key member of our community nursing team, you will be responsible for providing high-quality patient care and leading a team of nurses and healthcare assistants in delivering excellent care to patients in their own homes.
Key Responsibilities- Lead a team of nurses and healthcare assistants in delivering high-quality patient care
- Develop and maintain expertise in palliative nursing within the community setting
- Assess the holistic needs of patients and their families, considering physical, psychological, social, and spiritual needs
- Develop and implement care plans that meet the individual needs of patients
- Collaborate with other healthcare professionals to ensure seamless care delivery
- Contribute to the development of Hospice standards, policies, and research
- Registered Nurse with a valid UK nursing registration
- Experience of working in a community nursing setting, preferably in palliative care
- Excellent communication and interpersonal skills
- Ability to work effectively in a multi-disciplinary team
- Knowledge of local area and community resources
