Position: Social Worker - First Response Team

Compensation: £38 per hour (Umbrella)

The First Response Team offers a vital short-term service aimed at providing immediate assistance to individuals within the community. This role involves collaborating closely with various stakeholders, including the reablement service, Care Coordination Centre Triage Clinicians, and Community Services.

As a Social Worker within this team, your responsibilities will include:

  • Conducting assessments in accordance with the Care Act 2014 and the Mental Capacity Act 2005.
  • Engaging directly with clients, their families, and other professionals.
  • Developing Care Plans and managing short-term cases for adults requiring support.
  • Participating in Multi-Disciplinary meetings and performing initial Safeguarding inquiries.

This position is primarily based in the Bath and North East Somerset region, with a focus on providing services in the community.

To qualify for this role, candidates must possess:

  • A relevant Social Work qualification.
  • Registration with Social Care Wales.
