Position Overview

As a Community Engagement Specialist within the Age Well initiative, you will play a pivotal role in conducting thorough holistic evaluations of designated elderly individuals, primarily in their residences. Your objective will be to develop tailored care strategies that empower these individuals to thrive in their familiar surroundings, manage chronic health conditions, mitigate feelings of isolation, and enhance their overall wellbeing and quality of life.

To excel in this position, candidates should possess diverse experience, including direct clinical support in a healthcare environment, such as a Health Care Assistant or a comparable role. A genuine commitment to delivering effective assistance to our senior community members is essential.

A solid understanding of available community and hospital services, along with a keen interest in geriatric care, is crucial for ensuring that patients receive the necessary support. As a Community Engagement Specialist, you will facilitate referrals to guarantee that individuals in your area obtain exceptional and prompt assistance. Building strong collaborative relationships with other healthcare and social service providers is a vital component of this role. Each team member will have direct access to their local Befriending lead, as well as support personnel for Falls and Dementia.

Through additional training, if necessary, you will assess low-level equipment requirements and provide guidance for their procurement, as well as identify how technology can be leveraged to promote independence and assist in the self-management of chronic conditions.

Key Responsibilities

  • Initiate welfare outreach calls to individuals, including those discharged from A&E, to assess their needs.
  • Collaborate with General Practitioners and practice teams to identify elderly, frail individuals or those with long-term health requirements.
  • Organize, coordinate, and conduct initial home assessments to determine needs, making relevant referrals to local service providers.
  • Perform basic health evaluations, including monitoring weight, nutrition, blood pressure, temperature, and personal care requirements.
  • Assist individuals in the community to maintain their independence at home and reduce the likelihood of crisis situations. Tasks may include assessing and providing low-level mobility aids, organizing house clearance and cleaning, supplying essential items such as emergency food parcels, and supporting individuals with housing, financial matters, medication, and follow-up appointments.
  • Coordinate and participate in PCN Frailty Clinics.
  • Work collaboratively with families and a person's broader support network to ensure their preferences, needs, and support within their home are addressed as part of a long-term strategy.
  • Facilitate access to existing community groups and events.
  • Document and monitor aspects of patient coordination and service delivery, supporting data collection and audits using the clinical system and organizational data systems.

Qualifications and Experience

Essential

  • Experience as a Health Care Assistant or equivalent.
  • Prior experience within the NHS, either in a hospital or community setting.
  • Understanding of both physical and mental health needs.
  • Proficient in IT.
  • Able to work effectively within a team.
  • Working knowledge of a variety of therapy equipment and its safe application.
