Position Overview

In this role, you will be an integral part of a collaborative team, providing essential care and support to patients in their homes. You will have the flexibility to organize your daily schedule and make informed clinical decisions to benefit your patients.
Working Hours
The position requires availability from Monday to Sunday, with shifts ranging from 6 to 10 hours between 08:00 and 22:00. We prioritize a healthy work-life balance and are open to discussing your shift preferences.
Key Responsibilities
- Delivering clinical care, including wound management, palliative care, symptom management, bowel and continence care, catheter care, and medication administration.
- Collaborating with a multidisciplinary team to ensure comprehensive patient care.
- Assisting patients in maintaining their independence and avoiding unnecessary hospital admissions.
- Encouraging self-care practices among patients.
