Job Overview

An exceptional opportunity has emerged for a Lead Community Nurse to join the Out of Hours Team within the East Lancashire District Nursing Service.

This is a temporary part-time Band 6 role, ideal for a passionate and committed registered nurse with significant prior experience in District Nursing. The successful applicant will have the chance to thrive in a nurturing environment as a Team Sister, enhancing their skills in patient evaluation and care while contributing to team leadership.

The role requires flexibility across the District Nursing Service, with a primary focus on the out-of-hours night nursing service, operational from 20:00 to 08:15 nightly. Occasional daytime duties may be necessary to support your professional growth. This service operates seven days a week, including weekends and public holidays.

Previous experience in community nursing is a prerequisite.

Key Responsibilities

Collaborate with the District Nursing Team Manager to coordinate and deliver care aligned with both national and local District Nursing Strategies.

  • Conduct assessments for new and complex patients
  • Provide comprehensive Senior Nurse Reviews for patients in their homes and other community settings
  • Engage as part of a broader multidisciplinary team
  • Mentor students and new staff in their professional development
  • Foster team growth through effective leadership and role modeling
  • Manage staffing schedules and allocate clinical responsibilities
  • Establish and maintain clear communication with clinical staff, patients, and their families
  • Demonstrate the ability to work contracted hours across both day and night shifts as required by service needs.

The ideal candidate will exhibit professional leadership and a strong commitment to patient care. They should possess problem-solving skills, adaptability to challenging circumstances, efficient resource management, and a dedication to delivering safe, personalized, and effective patient care. A valid UK driving license and access to a vehicle are essential for this role.

Person Specification Qualifications Essential Criteria
  • Registered Nurse (RN Adult branch)
  • Community Specialist Practitioner Qualification or a willingness to pursue this qualification
Desirable Criteria
  • Non-medical prescriber V100/V150/V300
  • Leadership training
  • Level 6 study to enhance service delivery and development
  • Mentorship or Coaching qualification
Experience Essential Criteria
  • Minimum of 12 months substantial district nursing experience
  • Experience in holistic assessment of complex patients and care coordination
  • Experience in multidisciplinary team collaboration
Desirable Criteria
  • Experience in caseload management and work allocation
  • Experience in service development and supporting practice change
Knowledge and Skills Essential Criteria
  • Proficient in using information technology
