Job Overview

An exciting opportunity has emerged for a Community Nursing Team Leader to join the Out of Hours Team within the East Lancashire District Nursing Service.

This is a temporary part-time Band 6 role tailored for a passionate and skilled registered nurse with significant experience in community nursing. The successful applicant will have the chance to thrive in a nurturing environment as a Team Leader, enhancing their expertise in patient evaluation and care while playing a vital role in team leadership.

The position requires flexibility across the District Nursing Service, primarily focusing on the out of hours night nursing service, operational from 20:00 to 08:15 nightly. Occasional daytime responsibilities may arise to support your professional growth. This role encompasses a 7-day service, including weekends and public holidays.

Prior experience in community nursing is a prerequisite.

Key Responsibilities

The post holder will assist the District Nursing Team Manager in coordinating and delivering care in alignment with both national and local District Nursing strategies.

  • Assessing new and complex patient cases
  • Conducting comprehensive Senior Nurse Reviews for patients in their homes and other community settings
  • Collaborating within a broader multidisciplinary team
  • Mentoring students and new staff in their professional development
  • Fostering team growth through leadership and exemplary conduct
  • Managing staffing schedules and distributing clinical tasks
  • Establishing and maintaining effective communication with clinical staff, patients, and their families
  • Demonstrating the ability to work contracted hours across both day and night shifts as required by service needs.

The ideal candidate will exhibit professional leadership qualities and a deep commitment to patient care. They should possess problem-solving skills and adaptability in challenging situations, manage resources effectively, and ensure the provision of safe, personalized, and efficient patient care. We seek a nurse dedicated to delivering quality care and capable of constructively challenging practices to ensure the safety of both patients and staff.

A valid UK driving license and access to a vehicle are essential for this role.
