About You:

We are seeking a dedicated Community Nursing Team Leader to enhance our team in Bracknell. In this role, you will guide and support our community nursing staff, ensuring optimal patient care and outcomes.

Your Responsibilities:

  • Provide advanced clinical guidance and expertise to patients, applying evidence-based methodologies.
  • Facilitate a service aimed at preventing unnecessary hospital admissions and supporting patient discharge.
  • Collaborate with the Operational Team Lead to maintain staff proficiency.
  • Offer educational opportunities for both staff and students.

Key Duties:

  • Ensure care delivery is informed by thorough assessment and care planning.
  • Act as a representative for the service in various forums.
  • Oversee staff training and professional development.
  • Lead and manage a Community Nursing Team effectively.

  • Ability to function as a clinical authority
