Job Overview

Berkshire Healthcare NHS Foundation Trust is seeking a highly motivated and experienced Community Health Nurse to join our team. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care in the community, working closely with patients, families, and healthcare professionals to promote health and well-being.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their own homes, including assessment, planning, implementation, and evaluation of care.
  • Work collaboratively with multidisciplinary teams to develop and implement individualized care plans.
  • Conduct home visits to assess patients' needs and provide education on self-management and disease prevention.
  • Collaborate with community resources and healthcare providers to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • Registered General Nurse (RGN) qualification and current NMC registration.
  • Experience in community nursing, preferably in a similar role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to assess, plan, implement, and evaluate care, with a focus on patient-centered care.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
