Job Overview

Berkshire Healthcare NHS Foundation Trust is seeking a highly motivated and compassionate Registered Nurse to join our Community Nursing Team. As a Community Staff Nurse, you will play a vital role in delivering high-quality nursing care to patients in their own homes, promoting independence and improving health outcomes.

Key Responsibilities
  • Provide comprehensive nursing care to patients with varying healthcare needs, including end-of-life care, wound management, and more.
  • Conduct thorough assessments, develop care plans, and implement evidence-based interventions to improve patient outcomes.
  • Work collaboratively with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Participate in education and training programs to enhance your skills and knowledge, staying up-to-date with best practices and guidelines.
Requirements
  • Registered General Nurse (RGN) qualification and NMC registration.
  • Experience in community nursing, with a strong understanding of long-term conditions and self-management principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
  • Full UK driving license and access to a car, with the ability to travel across multiple sites in Berkshire.
