Job Summary:

As a Paediatric Community Nurse, you will be responsible for providing expert care and support to children with complex medical conditions, including Cornelia De Lange syndrome, ASD, and Bilateral profound sensorineural hearing loss. You will work closely with our multidisciplinary team to develop and implement individualized care plans, ensuring that each child receives the highest level of care and support.

Key Responsibilities:
  • Provide high-quality care and support to children with complex medical conditions
  • Develop and implement individualized care plans in collaboration with the multidisciplinary team
  • Work closely with families and carers to ensure that each child receives the highest level of care and support
  • Participate in regular team meetings and contribute to the development of care plans
  • Maintain accurate and up-to-date records of each child's care and progress
Requirements:

To be considered for this role, you will need to have:

  • A minimum of 6 months' experience in the last 2 years in a Paediatric Community Nursing role
  • Recent experience in Tracheostomy Care & Suction, Tracheostomy Tube Change, Ventilation via Tracheostomy, Humidification, Care & use of Gastrostomy, and Changing Gastrostomy Buttons
  • 12 months' minimum UK-based experience
