Job Overview

Berkshire Healthcare NHS Foundation Trust is seeking a skilled and compassionate Community Health Nurse to join our team. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care in the community, working closely with patients, families, and healthcare teams to promote health, prevent illness, and manage chronic conditions.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical and emotional needs, developing and implementing personalized care plans to address their unique requirements.
  • Provide expert nursing care to patients in their homes, community settings, and other locations, ensuring timely and effective interventions to prevent hospitalization and promote recovery.
  • Collaborate with multidisciplinary teams, including GPs, social workers, and other healthcare professionals, to coordinate care and ensure seamless transitions between settings.
  • Develop and maintain strong relationships with patients, families, and carers, providing education, support, and guidance to empower them to manage their health and well-being.
  • Stay up-to-date with the latest evidence-based practices, guidelines, and research, applying this knowledge to improve patient outcomes and inform service development.
Requirements
  • Registered General Nurse (RGN) qualification and current NMC registration.
  • Proven experience in community nursing, with a strong understanding of the complexities of community care and the ability to work independently.
  • Excellent communication, interpersonal, and organizational skills, with the ability to work effectively in a fast-paced environment.
  • Ability to travel across multiple sites in Berkshire, with a full UK driving license and access to a car.
  • Flexibility to work across 7-day service requirements, including evenings and weekends.
