Position Overview

Meir Primary Care Network is inviting applications for a skilled Clinical Associate Practitioner to join our healthcare teams.

This position is directly involved with patients, and the selected individual will be accountable for delivering clinical services, collaborating within a multidisciplinary team to provide care tailored to the patient demographic. The Clinical Associate Practitioner will operate independently within our healthcare facilities, offering a variety of services, including patient assessment, diagnosis, treatment, telephonic consultations, clinical judgment, and appropriate referrals.

The Clinical Associate Practitioner will function within their professional limits to deliver care for patients from the initial consultation and clinical evaluation through to diagnosis, treatment, and follow-up care.

Key Responsibilities

The individual in this role will:

  • Provide comprehensive care for patients from the initial consultation, clinical evaluation, diagnosis, treatment, and follow-up.
  • Exhibit safe clinical judgment and expert care, including assessment and diagnostic capabilities for patients receiving treatment.
  • Demonstrate analytical thinking in the decision-making process. Under the guidance of a supervising physician, collaborate as part of the primary care team to provide support that meets the needs of patients and their families, while adhering to established policies and procedures.

Clinical Duties

The Clinical Associate Practitioner will:

  • Deliver primary care services as the first point of contact for patients presenting with various health concerns, utilizing history-taking, physical examinations, and clinical judgment to establish a diagnosis and management plan in collaboration with the patient.
  • Support the management of chronic conditions through specialized clinics after receiving appropriate training.
  • Provide health promotion and preventive care advice, while analyzing and acting on diagnostic test results.
  • Foster integrated, patient-centered care by collaborating with the broader primary care team and social care networks.
  • Utilize clinical guidelines and advocate for evidence-based practices, participating in clinical audits and significant event reviews.
  • Conduct face-to-face, telephonic, and online consultations for both urgent and routine health issues.
  • Perform home visits as necessary.
  • Develop a personal development plan (PDP) through reflective practice, operating under suitable clinical supervision.

Training and Development

The Clinical Associate Practitioner will:

  • Stay informed on relevant medical research and evidence-based practices by attending educational courses and professional gatherings.
  • Commit to ongoing education and professional development to meet personal and professional growth needs.
  • Engage in various research and analysis tasks aimed at enhancing clinical care and medical diagnosis.
  • Participate in regular multidisciplinary meetings to discuss and learn from significant clinical events.
  • Reflect on personal practice to identify learning needs and promote self-directed lifelong learning.

Qualifications

Essential

  • Graduated from a recognized Physician Associate program and successfully completed the national examination.
  • Professional registration with the relevant governing body.
  • Experience working as a Clinical Associate Practitioner.
  • High level of accountability and autonomy in clinical practice.
  • Familiarity with a range of clinical conditions.
  • Understanding of research and audit processes.
  • Commitment to evidence-based practice.
  • Proven ability to develop effective working relationships.
