About the Role:

This is an excellent opportunity to work with a large multidisciplinary team and a diverse population. You will be responsible for working with service users to identify personal recovery goals, supporting individuals to navigate their care pathway, and promoting independent living.

Key Responsibilities:
  1. Work with the multi-disciplinary team to deliver support to individuals accessing the community service.
  2. Assume a coaching role supporting service users in developing personal recovery plans.
  3. Assist service users to identify their strengths, personal interests, and goals.
  4. Provide opportunities for service users to direct their own recovery process.
  5. Facilitate the individual to move through and beyond the community service.
  6. Model/mentor a recovery process and demonstrate coping skills.
  7. Act as a role model to service users to inspire hope and share life experiences.
  8. Support service users to negotiate an Advanced Directive with all people involved in their care.
  9. Contribute to the assessment, planning, implementation, and review of care with the multi-disciplinary team.
  10. Work together with the service user and their carers in drawing up care plans.
  11. Assist individuals in managing their mental health on a day-to-day basis.
  12. Positively promote and support independent living for service users in the community.
  13. Assist in the development and implementation of educational and peer-facilitated support groups and activities.
  14. Accompany service users to appointments and therapeutic or social community-based activities.
  15. Support service users in seeking to connect or reconnect with family, friends, and significant others.
  16. Assist service users to maintain a connection with their life outside of services.
Person Specification:

We are looking for an individual with a commitment to obtaining underpinning knowledge through work-based learning and mandatory training. Essential qualifications include a preparedness to work flexible hours and ability to travel between locations using own or public transport.

Desirable qualifications include an accredited certificate in Life Coaching and accredited peer support worker training.

Experience of working in a multidisciplinary team in health and social care is essential, as is personal lived experience of recovery from mental health challenges and personal experience of accessing secondary mental health services.

Desirable experience includes paid or unpaid peer support worker experience.

Skills and Knowledge:

We are looking for an individual with good interpersonal skills and ability to form peer relationships with service users and carers. Essential skills include effective listening skills, ability to communicate on all levels, and ability to consult and deliver training to other teams.

Desirable skills include basic Health and Safety awareness, Moving and Handling awareness, awareness of local services, knowledge of benefits/employment systems, and ability to demonstrate a good understanding of mental health issues.
