Medication Management Specialist

Position Overview
An exceptional opportunity has emerged for a Medication Management Specialist to become a vital member of our Primary Care Network (PCN). You will collaborate within a forward-thinking multidisciplinary team dedicated to enhancing the delivery of clinical pharmacy services across our network, supporting clinical pharmacists and practice colleagues in implementing the Clinical Pharmacy model of care.
The ideal candidate will receive guidance and support from our experienced PCN Clinical Pharmacists.

Your role will encompass a balance of patient interaction and clinical administrative tasks, focusing on:

Key Duties
Your responsibilities will include:
- Collaborating with Clinical Pharmacists and Clinicians to adhere to established prescribing protocols.
- Managing repeat medication requests and processing prescriptions in accordance with clinical guidelines.
- Engaging with patients to ensure effective medication use through shared decision-making.
- Conducting medication optimization tasks, including supporting medication reviews and ensuring accurate medicines reconciliation.
- Providing expert advice on public health and social care needs, including lifestyle modifications and addressing local health disparities.
- Participating in initiatives aimed at promoting responsible antibiotic use.
- Assisting in the implementation of national prescribing policies and conducting clinical audits to support quality improvement.
Technical and Administrative Responsibilities
You will work closely with the PCN team to ensure efficient medication management, including:
- Streamlining prescription processes and minimizing medication waste.
- Maintaining accurate patient records and ensuring compliance with local protocols.
- Providing training on safe medication handling and promoting the use of electronic prescriptions.
