About the Role

We are seeking a highly skilled and compassionate Palliative Care CNS to join our team at Hampshire Hospitals NHS. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients and their families in an acute hospital setting.

Key Responsibilities
  • Provide symptom management and psychological and social support to patients and their families in the acute hospital environment.
  • Deliver education to healthcare professionals, patients, and their families on palliative care and end-of-life care.
  • Work collaboratively with the community palliative care team to develop and deliver a comprehensive palliative care service.
  • Contribute to the development and implementation of policies and procedures related to palliative care.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Professional registration with the NMC.
  • Experience working with palliative care patients and in an acute hospital environment.
  • Additional post-registration learning in palliative care is desirable.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
