Job Title: Advanced Nurse Practitioner - Palliative Care Specialist

We are seeking an experienced Advanced Nurse Practitioner to join our team at Wessex Children's Hospice Trust (Naomi House), a leading UK charity providing palliative and end-of-life care for children and young people.

Job Summary

The Advanced Nurse Practitioner will be the primary point of contact for children and young adults with all types of health needs, working autonomously and in conjunction with other professionals to make decisions and provide appropriate nursing interventions.

Main Responsibilities
  • Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for children and young adults presenting with undifferentiated and undiagnosed conditions and complex needs.
  • Clinically examine and assess patient needs from a physiological and psychosocial perspective, and plan clinical care accordingly.
  • Proactively identify, diagnose, and manage treatment plans for children and young adults at risk of developing a long-term condition.
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan in liaison with the child, young adult, and family as appropriate.
Key Accountabilities
  • Practice as an Advanced Nurse Practitioner within own sphere of competence and agreed acceptable limits of practice, being accountable for own professional actions.
  • Be responsible for assessing, diagnosing, interpreting investigations, and treating children and young adults within the In-House Hospice service within a developing scope of competence.
  • Work as part of the multi-disciplinary team in providing quality evidence-based healthcare to meet the immediate needs of children and young adults.
Managerial and Clinical Responsibilities
  • Demonstrate commitment to developing the Advanced Nurse Practitioner role within the Organisation.
  • Act as a positive role model.
  • Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.
Personal and Professional Development
  • Be responsible and proactive in personal professional development by maintaining awareness of current developments in palliative care for children, young people, and families.
  • Maintain clinical skills identified in the Competency Framework.
  • Ensure timely revalidation with the NMC.
Person SpecificationExperience
  • Essential:
    • Experience of working as an Advanced Nurse Practitioner in a Ward environment.
    • Relevant experience of caring for children or young people or both with highly complex care needs and disability.
    • Experience of holistic assessment and individualised planning of care.
    • Experience of effective collaborative working with professionals from other agencies and disciplines.
  • Desirable:
    • Experience of working as an Advanced Nurse Practitioner in a Hospice environment.
    • NICU/PICU experience.
Interpersonal Skills
  • Essential:
    • Ability to cope with stressful situations and support staff in this.
    • Personal grief/loss resolved sufficiently to perform and cope in an environment that has likely exposure to bereavement concerns.
    • Willingness to work flexibly and adapt to changing service needs.
  • Desirable:
    • Enhanced communication skills including: Makaton, Augmentative and Alternative Communication (AAC), PECS.
Skills
  • Essential:
    • Competent clinical skills based on current best evidence, to include: medicine management; IV therapy; symptom management; respiratory support including invasive and non-invasive ventilation; nutritional care/enteral feeding; neurological care/seizure management.
    • Ability to gather information quickly, make decisions, and communicate appropriately.
    • Ability to manage own time and workload effectively.
  • Desirable:
    • Ventilation trained.

Qualifications
  • Essential:
    • REGISTERED NURSE with valid up-to-date registration with NMC.
    • RSCN/RN Child or Adult.
    • MSc in Advanced Clinical Practice qualification.
    • Independent & Supplementary Prescriber qualification.
    • Registration with NHSBSA as a Prescriber.
    • Clinical Examination and Diagnostics Level 7.
    • Eligibility to work as a Nurse in the UK and with UK current work permit if required.
