Job Summary

The postholder is responsible for delivering high-quality general practice nursing services to the entire practice population. Supported by senior nurses within the practice, they will provide care within the boundaries of their role. The post holder will make an assessment of a patient's needs and act upon their assessment.

Main Responsibilities
  • Assess, plan, develop, implement, and evaluate programs to promote health and well-being and prevent adverse effects on health and well-being.
  • Implement and evaluate individual treatment plans for patients with a known long-term condition.
  • Identify and manage as appropriate treatment plans for patients at risk of developing a long-term condition.
  • Prioritize health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including the initiation of effective emergency care.
  • Deliver opportunistic health promotion.
  • Provide information and advice on prescribed or over-the-counter medications, medication regimens, side effects, and interactions.
  • Work with patients to support adherence to prescribed treatments.
  • Assess and care for patients presenting with minor injuries.
  • Support and advise women requesting information relating to family planning needs.
  • Support and manage the health needs of women presenting for cytology.
  • Implement and participate in vaccination and immunization programs for both adults and children.
  • Advise, support, and where appropriate, administer vaccinations for patients traveling abroad.
  • Promote the use of Patient Group Directions (PGDs).
  • Promote and deliver evidence-based care for patients presenting with aural conditions.
  • Other tasks carried out by the practice nurse, which may include:
  • Dressing and wound management.
  • Phlebotomy.
  • ECG.
  • 24-hour BP/ECG.
  • Removal of clips and sutures.

Requirements
  • Registered Adult, Child, Mental Health, or Learning Disability Nurse.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme).
