About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Break Charity. As a Support Worker, you will play a vital role in providing high-quality care and support to young people with complex needs, including those with disabilities and those who have experienced trauma.

Key Responsibilities:

  • Provide individualized support to young people, promoting their physical, emotional, and psychological well-being.
  • Develop and maintain positive relationships with young people, their families, and other professionals.
  • Contribute to the creation and implementation of personalized care plans, ensuring the needs of each young person are met.
  • Participate in team meetings and contribute to the development of strategies to support young people's needs.
  • Collaborate with other professionals, including therapists and medical staff, to ensure comprehensive care and support.

Requirements:

  • Experience working with young people with complex needs, including those with disabilities and those who have experienced trauma.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with young people, families, and professionals.
  • Ability to work flexibly, including evenings, weekends, and sleepovers.
  • Commitment to providing high-quality care and support, with a focus on promoting the well-being and independence of young people.
