We are seeking an experienced and dynamic individual to join our Specialist Perinatal Outreach Team as a Perinatal Outreach Practitioner. The successful candidate will be responsible for providing outreach services to individuals in the perinatal period, working closely with the ward and community multidisciplinary team to identify patients who would benefit from our services.
Main Responsibilities- Perform home visits to assess and support patients in their own environment
- Manage and triage referrals into the ward and outreach team
- Act as a bed manager for the Andersen Ward Mother and Baby Unit
- Develop and implement care plans to support patients in the community
- Work closely with the multidisciplinary team to provide comprehensive care to patients
