The Senior Clinical Nurse Manager will oversee the administration and clinical guidance of the nursing team, which includes Nurses and Healthcare Assistants. This role encompasses the management and execution of various clinical domains such as infection control, health promotion, chronic illness management, and cervical screening. Additionally, the Senior Clinical Nurse Manager will collaborate with the management team to evaluate and implement clinical policies and procedures.
Key Responsibilities
The Senior Clinical Nurse Manager will be tasked with establishing processes for the effective management of patients with chronic conditions, utilizing evidence-based practices, particularly for elderly and housebound individuals.
All care programs for patients must be delivered in accordance with established guidelines.
As a senior member of the multidisciplinary team, the incumbent will ensure that nursing services are provided efficiently to the designated patient population.
Risk management within the areas of responsibility is crucial, ensuring that adequate measures are in place to safeguard both staff and patients. Regular monitoring of work environments and practices is essential to maintain safety and compliance with health, safety, and security regulations.

Core Responsibilities
The following are the primary responsibilities of the Senior Clinical Nurse Manager, with the understanding that additional tasks may arise based on workload and staffing levels:
- Act as the lead for Infection Prevention Control, ensuring compliance with relevant policies through audits and training.
- Prepare the annual Infection Prevention Control statement in collaboration with management.
- Serve as the CQC lead for the nursing team, ensuring that services are effective, safe, responsive, caring, and well-led.
- Assess patient needs to ensure appropriate healthcare provision, incorporating evidence-based practices.
- Develop and implement health promotion and wellness initiatives.
- Evaluate individual treatment plans for chronic disease patients to promote health and well-being.
- Identify and support patients at risk of developing long-term conditions.
- Provide routine nursing care in accordance with clinical evidence and guidelines.
- Diagnose and manage both acute and chronic conditions as required.
- Prescribe medications in line with established guidelines and personal scope of practice.
- Review patient medications to enhance adherence.
- Guide patients on the use of prescribed and over-the-counter medications.
- Coordinate travel medicine services.
- Request and process pathology services as necessary.
- Oversee chronic disease clinics, delivering patient care and referring to specialists as needed.
- Maintain accurate clinical records in compliance with current legislation.
- Ensure effective use of coding systems by all nursing team members.
- Maintain chronic disease registers appropriately.
- Ensure compliance with the practice chaperone policy.
- Prioritize health issues and intervene as necessary.
- Support the team in managing clinical emergencies.
- Recognize and refer patients with mental health needs.
- Lead vaccination programs for adults and children, collaborating with the team to enhance uptake.
- Assist patients in the use of their prescribed medications.
- Liaise with external services to ensure appropriate support for vulnerable patients.
- Delegate clinical responsibilities appropriately, ensuring safe practice.
- Support the clinical team with safeguarding matters in accordance with policies.
- Understand local policies regarding substance abuse and refer patients as needed.
- Deliver opportunistic health promotion when appropriate.
- Develop effective care management programs for frail and housebound patients.
- Be flexible to work outside of standard hours as required.
Additional Responsibilities
In addition to the primary responsibilities, the Senior Clinical Nurse Manager may be asked to:
- Develop and review audit protocols for the practice.
- Collaborate with partners and management to create nursing protocols.
- Support nursing team members through guidance and training needs assessments.
- Engage in local initiatives to enhance service delivery.
- Participate in shared learning to improve patient care.
- Continuously review clinical practices in response to national policies.
- Participate in the review of significant events using a structured approach.
- Manage the nursing team rota to ensure adequate staffing.
- Lead the planning and implementation of changes within the nursing team.
- Assist the practice team in reviewing and implementing policies and protocols.
