Lead Practice Nurse

Job Summary
The Lead Practice Nurse will be responsible for the management and clinical supervision of the practice nursing team, consisting of Nurses, HCAs, and other healthcare professionals. This role requires strong leadership and clinical skills to ensure the delivery of high-quality patient care.
Key Responsibilities
- Manage and deliver several clinical areas, including infection prevention and control, health promotion, chronic disease management, and cervical screening.
- Support the practice management team in reviewing and delivering clinical policy and procedure.
- Implement processes for the effective management of patients with long-term conditions using evidence-based practice.
- Ensure all programmes of care for patients are delivered under agreed guidelines (PGDs and PSDs).
Requirements
- Experience of working in a primary care environment.
- Experience of chronic disease management.
- Experience of working autonomously.
- Experience of working as a practice nurse or community nurse.
- Experience of infection prevention and control measures.
- Experience of managing elderly and housebound patients.
