The Lead Practice Nurse will be responsible for the strategic leadership and clinical supervision of the practice nursing team, ensuring the delivery of high-quality patient care and services. This role will involve managing and developing clinical areas such as infection prevention and control, health promotion, chronic disease management, and cervical screening.
Key Responsibilities- Act as the Infection Prevention Control (IPC) lead, ensuring compliance with IPC policies and procedures.
- Develop and implement health promotion and wellbeing programmes to improve patient outcomes.
- Assess patient needs and provide evidence-based care, incorporating chronic disease management and prevention strategies.
- Lead the development and implementation of individual treatment plans for chronic disease patients.
- Identify and manage patients at risk of developing long-term conditions, preventing adverse effects on patient health.
- Provide routine nursing care to patients, adhering to clinical guidelines and best practices.
- Diagnose and manage acute and chronic conditions, prescribing medication as necessary.
- Co-ordinate the provision of travel medicine services and oversee the management of chronic disease clinics.
- Maintain accurate clinical records and ensure the use of read codes for effective data retrieval.
Essential
- Experience of working in a primary care environment.
- Experience of chronic disease management and prevention.
- Experience of working autonomously and as part of a team.
- Registered Nurse (Nursing and Midwifery Council) qualification.
- GCSE grade A to C in English and Maths.
Desirable
- Experience of leading a nursing team in a primary care environment.
- Mentor or teaching qualification.
- Post graduate diploma or degree in Chronic Disease Management.
